Insurance Requirement

Students participating in activities are required to carry insurance. Medical care for injuries suffered by a Dexter student during the school day or at school activities remains the responsibility of the student's parent. If you do not have insurance, Myers-Stevens has insurance available for your purchase. An insurance policy may be purchased online at Myers-Stevens or by completing this form:

Physical Requirement

FREE Sports Physicals for Dexter athletes are provided each year, the day after the last day of school

Middle School athletes report to Middle School at 8:00 AM

Senior High athletes report to the Middle School at 10 AM

Bring the completed and (parent) signed, “Medical Examination for Participation in Interscholastic Athletics” form

Parent must answer all questions on the Health History (back of first page), with additional explanations as necessary.

If you wear glasses, bring them! If you fail the vision exam you will not pass the physical and will not be allowed to participate.

Please keep your form clean, dry and readable. If it cannot be read, it will not be signed by the examiners.
